Summer Day Camp

NOTICE: Due to construction at the Joplin Family YMCA, Summer Day Camp will be held OFF SITE.

Location: One Church (2802 New Hampshire Ave, in Joplin)
Check-In Time: 7:00 am – 8:30 am
Check Out Time: 3:30 pm – 6:00 pm
Breakfast, Lunch, and Snacks will be served.
Swim Days: Monday (5 & 6 year olds) Wednesday (7 & 8 year olds) Friday (9-12 year olds)
We will be accepting State Subsidy for this camp. Those using subsidy will need to call to enroll with the School Age Services Staff or Accounting Department.

Need Assistance: Parents/Guardians may get students registered by calling our School Age Services staff for online enrollment instructions @ 417-781-9622.

The Medication Authorization Form (To be completed by parent/guardian)
- Required fields must be completed:
  - Child’s Name, Parent/Guardian Signature, and Date
  - If your child is not on any medication, please type “No Meds”
The School-Age Child Health Report (To be completed by parent/guardian)
If applicable: Child’s IEP or Specialized Care Plan or Allergy Documentation
