Check Out Time:  3:30 pm – 6:00 pm

We will be accepting State Subsidy for this camp.  Those using subsidy will need to call to enroll with the School Age Services Staff or Accounting Department.

The Medication Authorization Form (To be completed by parent/guardian)

Required fields must be completed:

Child’s Name, Parent/Guardian Signature, and Date

If your child is not on any medication, please type “No Meds”

The School-Age Child Health Report (To be completed by parent/guardian)

If applicable:  Child’s IEP or Specialized Care Plan or Allergy Documentation
